Job summary

Sobe Innovative Rehab is seeking a licensed Physical Therapist to provide therapy services for hospice patients in a home setting in Homosassa, FL. This PRN role supports independence and quality of life with evidence-based care.

The position offers flexible PRN assignments, allowing you to choose when to work while maintaining clinical autonomy and working within a supportive team. Licensure in Florida is required, and new grads are welcome to apply.

Qualifications

• Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited program.
• Active Florida Physical Therapist (PT) license.
• New graduates and experienced therapists are encouraged to apply.

Responsibilities

• Evaluate and treat residents in an assisted living community and/or Home setting.
• Develop and implement individualized treatment plans.
• Collaborate with physicians, families, and the interdisciplinary care team.
• Maintain accurate, timely documentation in the EMR.
