Sobe Innovative Rehab is seeking a licensed Physical Therapist to provide hospice therapy in a home setting in Winter Haven, FL. Join our team to help older adults maintain independence and quality of life with compassionate, evidence-based care.

Responsibilities include evaluating and treating residents in home or assisted living settings, creating individualized treatment plans, collaborating with physicians and families, and maintaining EMR documentation.

Qualifications

• Licensed PT in Florida with an established credential.
• Ability to develop and implement individualized treatment plans.
• Comfortable providing home-based hospice therapy.
