Job summary

Sobe Innovative Rehab in The Villages, FL is seeking a licensed Physical Therapist (DPT/PT/RPT) to provide hospice therapy in a home setting on a PRN basis. You will evaluate and treat residents in their homes or assisted living communities, develop individualized plans, and collaborate with physicians and families.

This role offers flexible assignments, clinical autonomy, competitive pay, and the opportunity to make a meaningful impact while maintaining control of your schedule.

Qualifications

• Bachelor's, Master's, or Doctorate in Physical Therapy from an accredited program.
• Active Florida PT license.
• New graduates and experienced therapists are encouraged to apply.

Responsibilities

• Evaluate and treat residents in an assisted living community and/or home setting.
• Develop and implement individualized treatment plans.
• Collaborate with physicians, families, and the interdisciplinary care team.
• Maintain accurate, timely documentation in the EMR.
