Job summary

Sobe Innovative Rehab is seeking a licensed Physical Therapist to provide hospice therapy services in a home setting in Kathleen, FL. This PRN role offers flexible assignments and competitive compensation, with autonomy to manage your schedule.

Candidates must hold an active Florida PT license and have a degree in Physical Therapy. New graduates are welcome to apply and will cover patients in ZIP codes 33805 and 33801 as needed.

Qualifications

• Hospice/home-care experience preferred or willingness to work in home settings.
• Licensed Physical Therapist in Florida with current license required.
• New graduates and experienced therapists encouraged to apply (PRN).

Responsibilities

• Evaluate and treat residents in assisted living or home settings.
• Develop and implement individualized treatment plans.
• Collaborate with physicians, families, and interdisciplinary care team.
• Maintain accurate, timely documentation in the EMR.
