Sobe Innovative Rehab is seeking a licensed Physical Therapist to provide therapy services for Hospice Patients in a home setting. Join our team and help older adults maintain their independence and quality of life through compassionate, evidence-based care.

Responsibilities include evaluating residents in home or assisted living settings, creating individualized treatment plans, collaborating with the care team, and documenting in the EMR.

Qualifications

• Licensed Physical Therapist with Florida license
• New graduates and experienced therapists are encouraged to apply
• Able to cover patients in ZIP codes 33805 and 33801

• Develop and implement individualized treatment plans
• Collaborate with physicians, families, and the interdisciplinary care team
• Maintain accurate, timely documentation in the EMR

Education
