Job summary

Sobe Innovative Rehab in The Villages, FL is seeking a licensed Physical Therapist (DPT/PT/RPT) to provide hospice therapy for patients in a home setting. This PRN role offers flexible assignments and competitive compensation while helping older adults maintain independence and quality of life.

You will evaluate and treat residents in home or assisted living settings, develop individualized treatment plans, collaborate with physicians and families, and maintain accurate EMR documentation.

Qualifications

• Active Florida PT license.
• Graduate from an accredited PT program (DPT/PT/RPT).
• Ability to cover patients in ZIP codes 33805 and 33801.
• PRN availability and home-care setting experience preferred.

Responsibilities

• Evaluate and treat residents in assisted living or home settings.
• Develop and implement individualized treatment plans.
• Collaborate with physicians, families, and the interdisciplinary care team.
• Maintain accurate, timely EMR documentation.
