Sobe Innovative Rehab is seeking a licensed Physical Therapist to provide hospice therapy in a home setting in Tarpon Springs, FL. The role offers flexible PRN assignments with competitive compensation and clinical autonomy within a supportive team, helping older adults maintain independence and quality of life.

Responsibilities include evaluating residents, creating individualized treatment plans, coordinating with physicians and families, and maintaining EMR documentation.

Qualifications

• Valid Florida PT license
• Degree in Physical Therapy from an accredited program
• PRN schedule availability
