Sobe Innovative Rehab in Clearwater, FL is seeking a licensed Physical Therapist to provide hospice therapy services in a home setting. Join our team to help older adults maintain independence and quality of life through compassionate, evidence-based care.

This PRN role offers flexible assignments, clinical autonomy, and a supportive team. Must hold an active Florida PT license and a degree in Physical Therapy from an accredited program.

Qualifications

• Active Florida Physical Therapist (PT) license
• Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited program
• New graduates and experienced therapists are encouraged to apply

Responsibilities

• Evaluate and treat residents in an assisted living community and/or home setting
• Develop and implement individualized treatment plans
• Collaborate with physicians, families, and the interdisciplinary care team
• Maintain accurate, timely documentation in the EMR
