Job summary

Saint Joseph Health System is seeking a full-time Physical Therapist in Mishawaka, Indiana. You will assess and treat patients, develop individualized plans, and educate families on diagnoses and home exercises.

The team collaborates across disciplines to improve patient outcomes and support leadership development. Qualified candidates hold a Bachelor's degree in Physical Therapy, Indiana licensure, and current CPR certification.

Qualifications

• Bachelor's degree in Physical Therapy from an accredited program.
• Licensed Physical Therapist in Indiana.
• Current CPR certification.
• Clinical experience preferred; new graduates welcome.

Responsibilities

• Assess and treat patients referred to the Physical Therapy department
• Develop and implement individualized treatment plans based on evaluations and patient goals
• Educate patients and families on diagnoses, therapy plans, and home exercises
• Monitor progress, adjust treatment plans, and prepare discharge recommendations
• Document evaluations, progress notes, and discharge summaries accurately and timely
• Collaborate with interdisciplinary teams and participate in staff meetings and case management
• Supervise Physical Therapy Assistants, rehab techs, and volunteers
• Participate in departmental initiatives and student programs
