Job summary

Continuum Therapy Partners in Chicago, IL is seeking a qualified Physical Therapist for PRN daytime coverage at Norwood Crossing. You will deliver consistent care under the same clinical standards as our full-time team.

Required DPT, active IL license, NPTE, and up to 1 year of experience. You’ll evaluate, develop, and adjust individualized treatment plans, educate patients and families, and document progress to ensure continuity of care.

Qualifications

• DPT degree required.
• Active, unencumbered state Physical Therapy License; must pass NPTE.
• 1 year of previous experience as a Physical Therapist preferred.

Responsibilities

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.
• Documents patient progress and adjusts plans of care based on clinical outcomes.
• Educates patients and families on home exercise programs and injury prevention.

Physical Therapist Requirements & Responsibilities:

• Doctor of Physical Therapy (DPT) degree required
• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE)
