Northern Cardinal Rehabilitation and Nursing is seeking a dedicated Physical Therapist (PT) to join our resident-centered team in Virginia. You will assess referred patients, develop achievable rehab plans, and deliver direct care to improve functional outcomes in a geriatric population.

Collaborate with interdisciplinary staff, document progress, and participate in ongoing training to support quality patient-centered care while complying with HIPAA and safety standards.

Competenze

• Must be a graduate of an accredited Physical Therapy program.
• Must hold a valid PT license/registration for the state of practice.
• Experience in long-term care and geriatric care is preferred but not required.

Mansioni

• Assess referred patients requiring rehabilitation services.
• Provide direct patient care to assess condition, functional abilities, and rehab potential.
• Develop and administer treatment plans with goals aligned to patient capacity.
• Set realistic goals and document patient progress and outcomes.
• Educate patients and families and maintain accurate records of status and changes.
• Complete evaluations, progress notes, recertifications, and discharge documentation.
• Collaborate with interdisciplinary team to deliver patient-centered care.
• Perform duties as directed by Director of Rehabilitation.
• Ensure confidentiality, HIPAA compliance, and patient rights.
