Job summary

Northern Cardinal Rehabilitation and Nursing is seeking a Physical Therapist (PT) to deliver patient-centered rehabilitation in our long-term care setting. You will assess referred patients, develop and implement treatment plans, and collaborate with the care team to maximize function.

The role emphasizes documentation, progress notes, patient and family education, and compliance with HIPAA, with ongoing training in geriatrics and quality care.

Qualifications

• Graduated from an accredited Physical Therapy program.
• Valid license and registration for state of practice.
• Experience in long-term/geriatric care preferred.
• Experience with Case Mix Index and PPS scheduling preferred.

Responsibilities

• Assess referred patients requiring rehabilitation.
• Provide direct patient care to determine medical condition and rehab potential.
• Develop and administer a treatment program with physician direction.
• Set realistic goals aligned with patient capacity and tolerance.
• Document patient status and educate patients/families.
• Complete evaluations, progress notes, recertifications and discharge docs.
• Collaborate with all disciplines for patient-centered care.
• Perform duties assigned by Director of Rehabilitation and ensure HIPAA compliance.

Duties and Responsibilities:

• Responsible for the assessment of referred patients that require rehabilitation services.

• Providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation.

• Establish and administer a treatment program with specific goals determined according to the patients capacity and tolerance under the direction of the Physician.

• Set realistic and achievable goals for their patients

• Document and record the patients’ condition and educate patients and families in an appropriate physical therapy method.

• Evaluations, weekly progress notes, recertification and discharge documentations.

• Collaborates with all disciplines to provide best patient centered treatment and care.

• Perform all other duties assigned by Director of Rehabilitation

• Compliance with confidentiality, HIPPA and Federal Residents Rights.
  

Requirements/Qualifications:

• Must have graduated from an accredited Physical Therapy college or university.

• Must have a valid professional license and registration for state of practice.

• Experience with long-term care and/or geriatric care is preferred but not limited to.

• Experience with Case Mix Index and PPS scheduling is preferred however not limited to.
