Job summary

Eastern Health Group is seeking a Physical Therapist (PT) to assess and treat patients in our skilled nursing and rehabilitation facilities. You will develop individual treatment plans, set goals, and work with a multidisciplinary team to deliver quality rehab services.

The role emphasizes patient-centered care, proper documentation, and adherence to HIPAA and residents' rights, with opportunities for professional growth and supportive workplace culture.

Qualifications

• Graduated from an accredited Physical Therapy college or university.
• Must hold a valid professional license and registration for the state of practice.
• Experience with long-term care and/or geriatric care preferred.
• Experience with Case Mix Index and PPS scheduling preferred.

Responsibilities

• Assess referred patients requiring rehabilitation.
• Provide direct patient care to evaluate medical condition and functional capacity.
• Develop and administer treatment plans with physician guidance.
• Set realistic goals based on patient capacity and tolerance.
• Document patient status and educate patients and families.
• Prepare evaluations, progress notes, recertifications and discharge documents.
• Collaborate with disciplines to deliver patient-centered care.
• Perform other duties as assigned by Director of Rehabilitation.
• Ensure confidentiality, HIPAA compliance and patient rights.
