Enhance Rehabilitation is currently seeking qualified Physical Therapists to join our dynamic team. As a Physical Therapist, you will provide high-quality care for patients in SNF settings, evaluating and treating patients according to individualized care plans.

Responsibilities include:

1. Providing excellent care for our patients
2. Delivering outstanding customer service to patients and their families
3. Collaborating with facility staff to ensure optimal patient care
4. Evaluating new patients, developing care plans, and establishing measurable, functional goals
5. Treating patients, addressing concerns, and educating them about their injuries, progress, and expected outcomes
6. Supervising Physical Therapist Assistants
7. Re-evaluating patient progress, revising plans of care, and adjusting goals accordingly
8. Completing timely and accurate documentation, including evaluations, re-evaluations, discharges, and daily progress notes

Minimum requirements:
