Job summary

Concordia Lutheran Ministries is seeking a passionate Physical Therapist to join Concordia Visiting Nurses. Conduct comprehensive evaluations, develop personalized treatment plans, and empower patients to reach their maximum potential in home-health settings.

You will implement evidence-based interventions, monitor progress, educate families, collaborate with physicians, and help create a welcoming, supportive clinic environment where patients feel comfortable and motivated on their recovery

Qualifications

• Graduate from an accredited Physical Therapy program with a state license.
• Passion for patient-centered care and outcomes.
• Excellent communication and interpersonal skills.
• Team player who supports professional development and collaboration.

Responsibilities

• Conduct comprehensive evaluations and develop individualized treatment plans.
• Implement evidence-based interventions (manual therapy, therapeutic exercises, modalities).
