Job Details

• Facility: BayCare HomeCare
• Location: Tampa, FL
• Status: Full Time
• Shift Hours: 8:00 AM - 5:00 PM / Monday – Friday / 1 Weekend Rotation a month.
• Home visits in assigned service area: Tampa
• Weekend Work: Occasional
• On Call: Yes

Responsibilities

• Assists Physical Therapist with the administration of interventions utilized in physical therapy and rehabilitation.
• May progress patients in the treatment plan developed by the physical therapist.
• Communicates with all physicians involved in the plan of care and other healthcare practitioners (as appropriate) related to the current plan of care.
• Follows established procedures, emergency and safety precautions at all times in the treatment of patients and use of modalities.
• Maintains timely communications, records, reports, and other documentation in accordance with agency policies & procedures, and Medicare guidelines.
• Captures documentation during the home visit following company policy using company issued laptop.
• Assists in progressing therapy programs with prior assessments and approval, prepares clinical progress notes and participates case conference and utilization reviews.

Required

• Physical Therapist Assistant State of Florida Licensure
• Driver's License: State of Florida (to be obtained within 30 days of hire)
• Basic Life Support

Education

• Associate’s Degree in Physical Therapy Assistant

Experience

• 2 years Physical Therapy Assistant experience
• Preferred: 1 year Home Care
