Job summary

BayCare HomeCare in Sarasota, FL is hiring a Full-Time Physical Therapist to provide hands-on home health care. The role serves a defined service area, with visits in Sarasota County and a schedule from 8:00 AM to 5:00 PM, Monday through Friday, with occasional weekends and on-call duties.

• BLS certification required.
• PT license in Florida.
• Driver's license valid within 30 days of hire.
• Bachelor's degree in Physical Therapy required.
• Master's degree in PT preferred.
• 1 year of physical therapy experience.
• 1 year home care experience preferred.

Responsibilities

• Evaluate and develop patient-specific PT treatment programs.
• Provide clinical direction for PT Assistants.
• Plan and manage home health services to meet patient needs.
• Lead the multi-disciplinary care team.
• Monitor therapeutic services and communicate with care disciplines.
• Develop and execute discharge plans with patient and family.
• Explain agency policies to patients and families.

Responsibilities

The Home Health PT provides hands-on care in the home for a group of patients consistently in accordance with the principles and practices of physical therapy. Responsibilities include:

• Evaluates and develops specific treatment programs for individual patients based on their needs and in accordance with the principles and practices of Physical Therapy.
• Responsible for providing the appropriate level of clinical direction for Physical Therapy Assistants.
• Plans, implements, manages and evaluates the provision of both professional and ancillary home health services (direct care and case management) to ensure that all patient\'s needs are met and quality care provided in accordance with Federal, State, JCAHO and program guidelines.
• Manages the activities of the multi-disciplinary team providing care to the patient.
• Monitors the provision of therapeutic services through written and verbal communication with all disciplines.
• Develops, implements, and carries out a discharge plan in conjunction with the patient, family/caregiver and members of the health care team.
• Interprets agency policy to patients/families.
• Reviews patients visits weekly and provides plan to PCS and team assistant, including anticipated weekend visits.
• Informs team assistant of any schedule changes and communicates caseload and patient care issues to Patient Care Supervisor

Required

Certifications and Licensures:

• BLS (Basic Life Support)
• PT (Physical Therapist) License
• Driver's License State of Florida (Florida driver's license is required within 30 days of hire)

Education

• Required Bachelors (Physical Therapy)
• Preferred Masters (Physical Therapy)

Experience

• Required 1 year Physical Therapy
• Preferred 1 year Home Care
