Evolve Therapy Services is seeking a compassionate Physical Therapist Assistant for our in-house Skilled Nursing Facility in Annandale, VA. You will provide therapy services under a Physical Therapist's supervision and support resident evaluation and treatment planning.

Responsibilities include screening residents, assisting with evaluations, scheduling with the PT and team, and maintaining CMS-compliant documentation in NetHealth EMR.

Qualifications

• Active Physical Therapy Assistant license in state(s) of practice.
• Associate's degree in physical therapy from an accredited program.
• Ongoing continuing education to maintain licensure and certification.

Responsabilités

• Provide physical therapy services under supervision of a Physical Therapist.
• Screen residents regularly to assess need for intervention.
• Assist in evaluating residents and formulating treatment plans with the PT.
• Develop and modify resident schedules with the PT and care team.
• Document CMS-compliant records in NetHealth EMR.
