Job summary

American Senior Communities is seeking a Full Time Physical Therapist Assistant at Seymour Crossing in Seymour, IN. You will assist the licensed physical therapist, provide skilled care across ASC Therapies & Wellness Settings, including home health and outpatient services.

You'll collaborate with therapists, monitor patient progress, participate in discharge plans, and support case management while upholding state regulations and delivering compassionate, patient-centered care to our senior

FULL TIME - with ability to float to nearby campuses

The Physical Therapy Assistant assists in providing skilled care under the supervision of the registered physical therapist. Able to provide services for patients in all ASC Therapies & Wellness Settings. Able to provide services for patients in all ASC Therapies & Wellness Settings including home health, skilled nursing, outpatient therapies.

• Provide direct therapy to patients outlined in the OT's plan of care

• Collaborate with members for discharge plans
• Contribute to case management
• Maintain positive level of interaction with patients and center staff
• Adhere to applicable state regulations concerning occupational therapy
