• Assist licensed Physical Therapists in developing and implementing individualized therapy plans
• Conduct evaluations, assessments, and re-evaluations of patients to identify strengths, limitations, and goals
• Implement treatment plans, including exercises, modalities, and educational programs
• Communicate with patients, families, and healthcare providers to ensure seamless care coordination
• Maintain accurate records and documentation of patient progress
• Participate in interdisciplinary team meetings to share treatment plans and goals
