Spring Creek Healthcare Center, a premier 179-bed nursing home located in Perth Amboy, is seeking a dedicated Physical Therapist Assistant (PTA) to join our team. You will assist licensed PTs, conduct assessments, and implement evidence-based therapies to improve rehabilitation outcomes for residents.

As a PTA, you will play a vital role in enhancing the rehabilitation outcomes of our patients and residents, while working collaboratively with our multidisciplinary team.
Assist licensed physical therapists in providing high-quality patient care, focusing on achieving optimal functional outcomes
Conduct patient assessments and develop individualized treatment plans in accordance with patient goals and needs
Implement evidence-based interventions, including exercises, modalities, and other physical therapy techniques
Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination
Participate in patient education and discharge planning to promote independence and successful community reintegration
Maintain accurate and detailed records of patient treatment and progress
