Spring Creek Healthcare Center, a premier 179-bed nursing home located in Perth Amboy, is seeking a dedicated Physical Therapist Assistant (PTA) to join our team. You will assist licensed PTs, conduct assessments, and implement evidence-based therapies to improve rehabilitation outcomes for residents.

In this role, you will collaborate with a multidisciplinary team, document progress, and contribute to discharge planning to promote independence and community reintegration.

Qualifications

• Associate's degree or higher in Physical Therapist Assistant studies.
• Current New Jersey licensure as a Physical Therapist Assistant.
• Strong communication and interpersonal skills.

Responsibilities

• Assist licensed physical therapists in providing high-quality patient care, focusing on achieving optimal functional outcomes.
• Conduct patient assessments and develop individualized treatment plans in accordance with patient goals and needs.
• Implement evidence-based interventions, including exercises, modalities, and other physical therapy techniques.
• Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination.
• Participate in patient education and discharge planning to promote independence and successful community reintegration.
• Maintain accurate and detailed records of patient treatment and progress.
