Elderwood at North Creek in North Creek, NY is seeking a full-time Physical Therapist Assistant to support residents through skilled therapy under a Registered Therapist.

You will assist with treatment plans, document progress, and collaborate with an interdisciplinary team. Excellent communication, compassion, and license/certifications required; NY PTA license and CPR/BLS preferred.

Qualifications

• Graduate of an approved PTA program.
• New York State license to practice.
• Current CPR/BLS credentials.
• Strong communication and interpersonal skills.
• Team-oriented and compassionate.
• English reading/writing/speaking at intermediate level.

Responsibilities

• Assist the Physical Therapist in planning, developing, and carrying out treatment programs.
• Write observations of residents’ progress in progress notes.
• Maintain departmental records and other reports as requested.
• Assist with in-service training programs.
• Maintain supplies and sanitation of equipment.
• Attend scheduled shifts and manage own transportation.
• Utilize time-keeping system.
• Follow confidentiality and privacy standards.
• Follow dress code and wear name tag.
• Perform duties aligned with service excellence standards.
