Elderwood in New York is seeking a dedicated Physical Therapist Assistant to support residents with skilled therapies under supervision of a Registered Physical Therapist. The role emphasizes quality care and accurate progress documentation.

Responsibilities include planning and carrying out treatments, recording progress, and maintaining equipment and supplies. NYS PTA licensure and CPR/BLS are required, with strong English communication expected.

Qualifications

• Graduation from a physical therapy assistant approved program required.
• New York State license to practice required.
• Current, valid CPR/BLS credentials.
• English language reading/writing at an intermediate level or higher.

Responsibilities

• Assist the Physical Therapist in planning, developing and carrying out treatment programs for residents.
• Writes observations of residents’ behavior and progress in Progress Notes.
• Maintains statistical records and departmental reports as requested by the Physical Therapist.
• Assist with in-service training programs upon request.
• Ensure daily sanitation of equipment and proper supply management.
• Arrive on time and maintain regular attendance; coordinate transportation as needed.
• Follow facility dress code and privacy requirements.
• Perform duties per service excellence standards established for the position.
