Concept Rehab is hiring a Physical Therapist Assistant (PTA) for a Skilled Nursing Facility role. Under a supervising Physical Therapist, you will deliver treatments, help patients regain mobility, and communicate progress with the care team.

Qualifications

• Graduate of an accredited Physical Therapist Assistant program.
• Associate degree as a Physical Therapist Assistant.

Responsibilities

• Deliver physical therapy treatments according to established plans of care.
• Help patients improve mobility, strength, balance, function, and independence.
• Communicate patient progress with supervising PT and care team.
• Collaborate with physicians, nursing staff, and families to support outcomes.
• Complete timely documentation, notes, and daily billing activities.
• Assist with discharge planning and continuity of care.
• Support restorative and functional maintenance programs in the facility.

Requirements

What You'll Do

Under the supervision of a Licensed Physical Therapist, you will:

• Deliver physical therapy treatments according to established plans of care
• Help patients improve mobility, strength, balance, function, and independence
• Communicate patient progress and concerns with the supervising Physical Therapist and interdisciplinary care team
• Collaborate with physicians, nursing staff, caregivers, and families to support patient outcomes
• Participate in care planning and interdisciplinary team meetings as appropriate
• Provide education and training to patients, families, and facility staff
• Complete timely electronic documentation, treatment notes, and daily billing activities
• Assist with discharge planning and continuity of care
• Support restorative and functional maintenance programs within the facility
• Maintain productivity and documentation standards while delivering high-quality patient-centered care

Qualifications

• Graduate of an accredited Physical Therapist Assistant program
• Associate degree as a Physical Therapist Assistant
• Current PTA license in the state of practice
• New graduates are encouraged to apply
