Job summary

Concept Rehab & Engage Consulting in Ohio is seeking a Physical Therapist Assistant for a Skilled Nursing Facility to support patient mobility and independence under a licensed PT. You will work within an interdisciplinary team to deliver care and participate in planning and education for patients and families.

Under supervision, you will document, bill, and contribute to discharge planning while maintaining productivity and high-quality care.

Qualifications

• Graduate of an accredited Physical Therapist Assistant program
• Associate degree as a Physical Therapist Assistant
• Current PTA license in the state of practice
• New graduates encouraged to apply
• Strong communication, teamwork, and clinical problem-solving skills
• Passion for delivering exceptional patient-centered care in a Skilled Nursing Facility setting

Responsibilities

• Deliver physical therapy treatments according to established plans of care
• Help patients improve mobility, strength, balance, function, and independence
• Communicate patient progress and concerns with the supervising Physical Therapist and interdisciplinary care team
• Collaborate with physicians, nursing staff, caregivers, and families to support patient outcomes
• Participate in care planning and interdisciplinary team meetings as appropriate
• Provide education and training to patients, families, and facility staff
• Complete timely electronic documentation, treatment notes, and daily billing activities
• Assist with discharge planning and continuity of care
• Support restorative and functional maintenance programs within the facility
• Maintain productivity and documentation standards while delivering high-quality patient-centered care
