Independence Rehab is seeking a compassionate PRN Physical Therapist Assistant (PTA) to support our therapy team at Park Forest Care Center. At poresent, we need 5-10 hours of patient care per week.

In skilled nursing, small progress creates life-changing impact. Our goal is to help each patient regain mobility, confidence, and independence so they can safely transition to the next level of care.

What You'll Do

As a PRN PTA, you'll provide hands-on, functional therapy to a geriatric population, including:

• Gait and mobility retraining
• Transfer training across all levels of assistance
• Strengthening, balance, and fall prevention
• Supporting patients with medically complex conditions
• Collaborating with the supervising PT on care progression
• Working alongside nursing, OT, and speech to support whole-person recovery
