In House Rehab Team
Per Diem Position
Springfield, MA

Provides physical therapy services to residents under the direct supervision of a physical therapist, including assisting in assessment, treatment, program planning and implementation, related documentation, and communication.

Education, Experience, and Licensure/Certifications

• Must have an active state license or be license eligible and meet the educational requirements as defined in the job description for that license type.
• Associate Degree or Higher from an accredited Physical Therapist Assistant program.

Responsibilities include but are not limited to:

• Provides physical therapy services to residents under the direct supervision of a physical therapist.
• Identifies indications for and assists in evaluations at appropriate intervals under the direction of the physical therapist.
• Assists the physical therapist in identifying a problem list, short-term goals, long-term goals, and treatment plan.
• Maintains and completes accurate documentation, department records, and reports.
