Job summary

Elite Home Health is seeking a dedicated Physical Therapist Assistant (PTA) to deliver skilled therapy services to patients in their homes across Trenton, FL and surrounding areas. Under the direction of a licensed Physical Therapist, you will implement the therapy plan of care and ensure compliance with regulations.

Qualifications

• Graduate of an accredited Physical Therapist Assistant program.
• Current Florida PTA license/certification in good standing.
• CPR certification required.
• Home health experience a plus, clinical experience preferred.

Responsibilities

• Provide skilled PTA services under the supervision of a licensed PT.
• Assist patients with therapeutic exercises, gait and mobility training.
• Support fall prevention and safe functional movement.
• Monitor patient response and report to the PT.
• Document accurately in EMR and communicate with care team.

We are seeking a dedicated Physical Therapist Assistant (PTA) to provide skilled therapy services to patients in their homes under the direction and supervision of a licensed Physical Therapist (PT).

Position Summary

The PTA provides skilled physical therapy treatments in accordance with physician orders, the established therapy plan of care, PT direction, agency policies, and applicable regulations.

Responsibilities

• Provide physical therapy treatments under the direction of the supervising PT
• Assist patients with therapeutic exercises, gait training, transfer training, balance activities, and mobility training
• Support strengthening, fall prevention, and safe functional movement
• Monitor patient response to treatment and report progress or concerns to the PT
• Reinforce patient and caregiver education as directed by the PT
• Complete accurate and timely documentation in the EMR
• Communicate with the PT and interdisciplinary care team regarding patient needs
• Participate in care coordination, meetings, in-services, and required training
• Follow Medicare requirements, state regulations, and agency policies

Qualifications

• Graduate of an accredited Physical Therapist Assistant program
• Current Florida PTA license/certification in good standing
• Previous clinical experience preferred; home health experience a plus
• Current CPR certification
• Valid driver’s license, reliable transportation, and auto insurance
• Ability to meet all required background and compliance requirements
