Acloché LLC is seeking a passionate Physical Therapist Assistant to provide in-home care in San Francisco, CA. You will deliver skilled PT interventions and develop personalized treatment plans to improve patients’ mobility and quality of life.

You will collaborate with a multidisciplinary team to ensure holistic patient care, maintaining compliance with CA licensing and immunization requirements. The role involves home visits within the San Francisco area.

Qualifications

• Current Physical Therapist Assistant license in California.
• Valid CA Fingerprint Clearance Card.
• BLS (CPR) / ACLS certification.
• Recent Negative TB test and Flu Shot within the past year.
• Immunization records including Covid19, Hep B, MMR, TDaP, Varicella.
• Valid Driver's License along with auto-insurance.

Responsabilités
