Qualifications

• Licensed Physical Therapist Assistant in California.
• Current CA Fingerprint Clearance Card.
• Active BLS and ACLS certifications.
• Recent TB test and Flu shot within past year.
• Immunization records including Covid19, Hep B, MMR, TDaP, and Varicella.
• Valid driver’s license and auto-insurance.

Required Qualifications

• Current Physical Therapist Assistant license in the State of California.
• Valid CA Fingerprint Clearance Card.
• Up-to-date BLS (CPR) / ACLS certification.
• Recent Negative TB test result and Flu Shot within the past year.
• Comprehensive Immunization/Vaccination Records, including Covid19, Hep B, MMR, TDaP, and Varicella.
• Valid Driver's License along with auto-insurance.
