Food Management Search, Inc. is seeking a passionate Physical Therapist Assistant to join our team. You will provide in-home PT services in San Francisco, CA, delivering personalized therapy to improve patients' quality of life.

Responsibilities include delivering skilled interventions, creating individualized plans, and collaborating with a multidisciplinary team to ensure comprehensive care for patients in their homes.

Formación

• Licensed Physical Therapist Assistant in CA.
• CA Fingerprint Clearance Card required.
• BLS/ACLS certification required.
• Recent TB test result and Flu shot within past year required.
• Immunization records including Covid19, Hep B, MMR, TDaP, Varicella required.
• Valid driver’s license with auto-insurance required.

Key Responsibilities

• Delivering skilled physical therapy interventions, including exercises and stretches, aimed at enhancing patients' mobility and function.
• Creating and executing individualized treatment plans tailored to each patient's specific needs and goals.
• Collaborating with a multidisciplinary team of healthcare professionals to ensure holistic and coordinated patient care.

Required Qualifications

• Current Physical Therapist Assistant license in the State of California.
• Valid CA Fingerprint Clearance Card.
• Up-to-date BLS (CPR) / ACLS certification.
• Recent Negative TB test result and Flu Shot within the past year.
• Comprehensive Immunization/Vaccination Records, including Covid19, Hep B, MMR, TDaP, and Varicella.
• Valid Driver's License along with auto-insurance.
