Job summary

Transition Home Healthcare in Houston, TX is seeking a dedicated Physical Therapist Assistant (PTA) to support patients in their homes. The role offers PRN and full-time shifts in the Southeast service areas, with competitive benefits and growth opportunities.

The PTA will assist in rehabilitation, implement care plans, coordinate with the home health team, educate patients and families, and document care accurately. Strong communication and a valid Texas PTA license are required.

Qualifications

• Licensed Assistant Physical Therapist in Texas.
• Home health experience preferred.
• Knowledge of home health regulations and compliance.
• Experience with HCHB EMR (preferred).
• Able to handle physical workload.
• Good driving record.
• Excellent communication and people skills with patients and teams.

Responsibilities

• Assist in rehabilitation of patients in home care.

Responsibilities:

• Assist in the facilitation of rehabilitation of patients with emotional, physical or mental disabilities.
• Assist in the implementation of physical therapy care plans.
• Assist with the coordination of care with the home healthcare team.
• Educate patients and family members about health conditions.
• Prepare timely, complete, and accurate clinical notes.
• Communicate effectively with patients, caregivers, healthcare providers, office support, and other care team members.

Qualifications:
