Continuum Therapy Partners, LLC is seeking a dedicated Physical Therapist Assistant to split time between Trinity Care Center in Farmington and Three Links Care Center in Northfield, Minnesota. This full-time PTA role supports a collaborative rehab team in skilled nursing settings, delivering quality care to residents across two campuses.

You will partner with therapists to implement treatment plans, document progress, and promote functional recovery.

Qualifications

• Minnesota PTA license required.
• Experience in rehabilitation settings preferred.
• Strong teamwork and communication skills.

Responsibilities

• Assist licensed therapists in implementing treatment plans.
• Monitor patient progress and document outcomes.
• Collaborate with care staff to support resident recovery.
