Job Juncture is seeking a passionate Physical Therapist Assistant to provide in-home therapy in San Jose, CA. You will deliver skilled interventions, assess and adjust treatment plans, and work with a multidisciplinary team to ensure high-quality, coordinated care for diverse patients.

The role requires a California PTA license, fingerprint clearance, current BLS/ACLS, recent TB and flu vaccination, and comprehensive immunization records.

Qualifications

• Current Physical Therapist Assistant license in the State of California.
• Valid CA Fingerprint Clearance Card.
• Up-to-date BLS (CPR) / ACLS certification.
• Recent Negative TB test result and Flu Shot within the past year.
• Comprehensive Immunization/Vaccination Records, including Covid19, Hep B, MMR, TDaP, and Varicella.
• Valid Driver's License along with auto-insurance.
