Job summary

Interim HealthCare is seeking a Home Health Physical Therapist Assistant to provide one-on-one, home-based therapy focused on restoring mobility and function. You will work within a home health team to implement and adjust plans of care tailored to each patient’s daily life.

Ideal candidates have PTA education, SC licensure, and at least 1 year of experience, preferably in home health, with strong clinical judgment and compassion.

Qualifications

• Graduate of an accredited PTA program with active SC license.
• Minimum of 1 year PTA experience, ideally in home health.
• Knowledge of state and federal home health regulations.

Responsibilities

• Provide home-based physical therapy to patients with mobility limitations.
• Collaborate with a home health team to meet the plan of care and goals.
• Observe, assess, document progress, and adjust care plans as needed.
• Educate patients on exercises and self-care.
• Coach family on the plan of care and safety during exercises.

As a Home Health Physical Therapist, here’s a big-picture view of what you’ll do:

• Provide home-based physical therapy to patients who have lost mobility, strength and functionality due to an illness or injury


• Work as part of a home health team which may include an RN, LPN, CNA, PT,,OT and SLP, focused on the patient’s plan of care and goals


• Observe movements, conduct assessments, document progress and modify care plan as needed


• Assist patient with exercises and hands-on therapy to manage pain and increase mobility


• Educate patients on their plan of care, established goals, exercises and self-care


• Coach family on the plan of care, therapy goals and safely assisting with exercises

A few must-haves for Home Health Physical Therapists:

• Graduate of an accredited Physical Therapy Assistant Program and active PTA license in South Carolina


• Minimum of 1 year of physical therapy experience, ideally in home healthcare


• Knowledge of state and federal home health regulations


• Good clinical judgment, strong interpersonal skills, dexterity and compassion
