Job summary

Interim HealthCare of the Midlands is seeking a Home Health Physical Therapist (PT) to provide skilled, compassionate care in patients' homes across South Carolina. You will empower patients to recover, manage chronic conditions, and live safely at home while preventing hospitalizations and improving long-term outcomes through trusted relationships and coordinated care.

• Active South Carolina PT license required, new grads welcome.

Responsibilities

• Assesses patient function and equipment needs; develops plan of care with goals and interventions.
• Completes initial and ongoing comprehensive assessments as scheduled.
• Participates in ongoing interdisciplinary assessment of the patient.
• Provides physical therapy services ordered by the physician as indicated in the plan of care.

WhatYou’llDo

As a Home Health Physical Therapist (PT), you will:

• Assists the physician in evaluating the level of patient function and performance and equipment needs as well as in developing the resulting plan of care/treatment including appropriate long and short-term treatment goals, selection of interventions, and the intensity, frequency and duration of services to meet the needs of an individual patient.
• Completes initial and ongoing comprehensive assessments at appropriate time points as assigned.
• Participates in ongoing interdisciplinary assessment of the patient.
• Provides physical therapy services that are ordered by the physician as indicated in the plan of care.
