Job summary

Resolve Diamondback is seeking a dedicated Physical Therapist Assistant (PTA) to join our rehabilitation team in Phoenix, Arizona. We support residents in regaining mobility, independence, and achieving rehabilitation goals, welcoming both experienced PTAs and new graduates.

Responsibilities include under supervision delivering treatments, monitoring progress, maintaining documentation, and educating residents and families on exercises and safety, while collaborating with nursing and therapy

Qualifications

• Active Arizona PTA license is required.
• Graduate of an accredited PTA program.

Key Responsibilities

Provide physical therapy treatments under the direction and supervision of a licensed Physical Therapist.

Assist residents in improving strength, mobility, balance, and functional independence.

Implement individualized treatment plans established by the supervising Physical Therapist.

Monitor resident progress and communicate changes to the therapy team.

Maintain accurate and timely documentation in accordance with facility and regulatory requirements.

Collaborate with nursing, therapy, and interdisciplinary teams to support positive resident outcomes.

Educate residents and families on therapeutic exercises, mobility techniques, and safety.

Qualifications

Active Arizona Physical Therapist Assistant (PTA) license required.

Graduate of an accredited Physical Therapist Assistant program.

Previous skilled nursing, long-term care, or rehabilitation experience preferred.
