Job summary

Phoenix Home Care and Hospice seeks a Physical Therapist Assistant to provide in-home patient care under the supervision of a licensed PT. You will gather information and collaborate to adjust the care plan, helping patients achieve mobility goals and maintain independence.

Our team uses AI-powered charting during visits to boost efficiency and work-life balance. MO PTA certification and Home Health experience are preferred; strong teamwork and compliance are required.

Qualifications

• MO PTA certification required to practice in Missouri.
• Home health experience preferred but not required.
• Ability to pass drug test, background check, and physical.

Responsibilities

• Provides PT assigned services to patients in the home setting under supervision of a licensed PT.
• Participates in communication and case conferences to promote continuity and quality of care.
• Demonstrates sound judgment and technical skills per agency protocols.
• Maintains compliance with the Agency Code of Ethics and state/federal regulations.
• Practices safety, infection control, and other required measures.
• Performs miscellaneous duties as assigned by Supervisor.

Responsibilities

• Provides PT assigned services to patients in the home setting in accordance with Physician plan of care and under the supervision of a licensed PT.
• Participates in communication and case conference activities to promote maximum continuity and quality of patient care services.
• Exhibits sound judgement and technical skills necessary to perform assigned treatment and procedure according to established agency protocols.
• Maintains compliance with the Agency Code of Ethics.
• Demonstrates ongoing compliance with agency standards, policies, procedures, state and Federal regulations, state practice acts for licenses personnel, payer requirements.
• Practices safety, environmental, and/or infection control measures.
• Performs miscellaneous job-related duties as assigned by Supervisor.

Requirements

• Physical Therapist Assistant Certification for the state of MO
• Home Health experience preferred, not required.
• Ability to Pass Drug Test, Background Check, and Physical
• Multi-disciplinary collaboration for patient care
