Job summary

Phoenix Home Care & Hospice is seeking a Physical Therapist Assistant to provide home health PT services in Independence, MO and surrounding areas, under the supervision of a licensed Physical Therapist.

Requirements include MO PTA certification; Home Health experience preferred; ability to pass standard checks. This is a full-time role with benefits and ongoing training.

Qualifications

• MO PTA certification is required.
• Home Health experience preferred, not required.

Responsibilities

• Provides PT assigned services to patients in the home setting under the supervision of a licensed PT.
• Participates in communication and case conferences to promote continuity and quality of patient care.
• Maintains compliance with agency policies, procedures, and regulations.
• Performs duties with safety and infection control measures.
• Completes charting and documentation during visits with AI-assisted support.

Responsibilities

• Provides PT assigned services to patients in the home setting in accordance with Physician plan of care and under the supervision of a licensed PT.
• Participates in communication and case conference activities to promote maximum continuity and quality of patient care services.
• Exhibits sound judgement and technical skills necessary to perform assigned treatment and procedure according to established agency protocols.
• Maintains compliance with the Agency Code of Ethics.
• Demonstrates ongoing compliance with agency standards, policies, procedures, state and Federal regulations, state practice acts for licenses personnel, payer requirements.
• Practices safety, environmental, and/or infection control measures.
• Performs miscellaneous job-related duties as assigned by Supervisor.

Requirements

• Physical Therapist Assistant Certification for the state of MO
• Home Health experience preferred, not required.
• Ability to Pass Drug Test, Background Check, and Physical
• Multi-disciplinary collaboration for patient care
