Job summary

Phoenix Home Care & Hospice is seeking a part-time/PRN Physical Therapist Assistant to provide in-home therapy services in Springfield, MO. You will work under a licensed PT to help patients reach mobility goals and maintain independence while leveraging AI-assisted charting for better work-life balance.

Qualifications

• Physical Therapist Assistant Certification for MO is required.
• Home Health experience preferred.
• Ability to pass drug test, background check, and physical.
• Multi-disciplinary collaboration for patient care.

Responsibilities

• Provides PT assigned services to patients in the home setting under the supervision of a licensed PT.
• Participates in communication and case conferences to promote continuity and quality of patient care.
• Exhibits sound judgement and technical skills to perform treatments per agency protocols.
• Maintains compliance with the Agency Code of Ethics.
• Demonstrates compliance with agency standards, policies, and regulations.
• Performs safety, infection control measures, and other duties as assigned.

Responsibilities

• Provides PT assigned services to patients in the home setting in accordance with Physician plan of care and under the supervision of a licensed PT.
• Participates in communication and case conference activities to promote maximum continuity and quality of patient care services.
• Exhibits sound judgement and technical skills necessary to perform assigned treatment and procedure according to established agency protocols.
• Maintains compliance with the Agency Code of Ethics.
• Demonstrates ongoing compliance with agency standards, policies, procedures, state and Federal regulations, state practice acts for licenses personnel, payer requirements.
• Practices safety, environmental, and/or infection control measures.
• Performs miscellaneous job-related duties as assigned by Supervisor.

Requirements

• Physical Therapist Assistant Certification for the state of MO
• Home Health experience preferred, not required
