Job summary

Phoenix Home Care and Hospice seeks a licensed Physical Therapist Assistant to deliver home health PT services under a supervising PT. This PRN role supports mobility goals and patient independence in Bolivar and surrounding areas.

The position emphasizes safe practice, collaboration with the PT, and charting with Roger's AI-powered support to balance workload and work-life balance.

Qualifications

• Physical Therapist Assistant Certification for MO.
• Home Health experience preferred, not required.
• Ability to Pass Drug Test, Background Check, and Physical.
• Multi-disciplinary collaboration for patient care.
• Maintains compliance with Agency standards, policies, procedures, state and Federal regulations.

Responsibilities

• Provides PT assigned services to patients in home setting under supervision of a licensed PT.
• Participates in communication and case conferences to promote continuity and quality of care.
• Exhibits sound judgement and technical skills for treatments according to agency protocols.
• Maintains compliance with Agency Code of Ethics.
• Demonstrates compliance with agency standards, policies, and state/federal regulations.
• Practices safety and infection control measures.
• Performs miscellaneous duties as assigned by Supervisor.

Responsibilities

• Provides PT assigned services to patients in the home setting in accordance with Physician plan of care and under the supervision of a licensed PT
• Participates in communication and case conference activities to promote maximum continuity and quality of patient care services
• Exhibits sound judgement and technical skills necessary to perform assigned treatment and procedure according to established agency protocols
• Maintains compliance with the Agency Code of Ethics
• Demonstrates ongoing compliance with agency standards, policies, procedures, state and Federal regulations, state practice acts for licenses personnel, payer requirements
• Practices safety, environmental, and/or infection control measures
• Performs miscellaneous job-related duties as assigned by Supervisor.
