Job summary

Juniper Gardens Center for Nursing & Rehabilitation is seeking a licensed Physical Therapist Assistant (PTA) for a full-time, day shift in a Long-Term Care / Skilled Nursing Facility. You will work on a supportive rehab team to provide hands-on care and help residents reach therapy goals.

Responsibilities include following PT plans, offering individual and group therapy, tracking notes and billing, and assisting with discharge planning and patient education.

Qualifications

• Graduate of an accredited PTA program.
• Current state licensure as a PTA.
• Prior LTC/SNF experience is a plus, but not required.
• Ability to lift up to 50 lbs and assist residents as needed.
• Strong teamwork and time management skills.
