Job summary

Continuum Therapy Partners is seeking Home Health Physical Therapist Assistants in Milwaukee, WI and surrounding areas to join our growing home health team. We offer flexible full-time and PRN roles with the freedom to create your own schedule and provide one-on-one patient care.

You will work under a supervising Physical Therapist, use your Associate Degree to implement plans, document progress, and educate patients on home exercise programs.

Qualifications

• Associate Degree in Physical Therapist Assistant Program required.
• Current, unencumbered Wisconsin PTA license.
• 1 year of PTA experience preferred.
• Implements individualized treatment plans under supervision of a Physical Therapist.
• Educates patients on home exercise programs and assistive devices.

Responsibilities

• Implements individualized treatment plans under the supervision of a Physical Therapist.
• Documents patient progress and reports changes in status to the supervising PT.
• Educates patients on home exercise programs and proper use of assistive devices.

Physical Therapist Assistant Requirements & Responsibilities:

• Associate Degree in Physical Therapist Assistant Program required
