Concierge Home Care is seeking a Home Health Physical Therapist Assistant (PTA) for Bradenton, FL. You will deliver hands-on, one-on-one PTA services in patients' homes, following a plan of care set by a supervising PT.

Qualifications

Your Role as a Home Health Physical Therapist Assistant

• Provide hands-on therapeutic care following the plan of care established by the Physical Therapist
• Assist patients in improving mobility, strength, and independence
• Collaborate with patients, caregivers, and interdisciplinary teams
• Monitor patient progress and communicate updates to the supervising PT
• Complete documentation using streamlined, AI-assisted tools

Qualifications
