Job summary

Concierge Home Care is seeking a Home Health Physical Therapist Assistant (PTA) to support Bay County and surrounding areas in a flexible, home-based setting. Earn $45-$50 per visit based on experience, with autonomy to design your daily schedule and 1:1 patient care.

You’ll work with a supervising PT to implement the plan of care, document care using AI-assisted tools, and enjoy a proactive growth environment with mentorship and ongoing clinical education.

Qualifications

• Active Florida PTA license is required or eligible for licensure in Florida.
• Valid driver’s license with reliable transportation.
• Home health experience preferred but not required.
• Ability to work independently in a home-health setting.

Responsibilities

• Provide hands-on therapeutic care following the plan of care established by the supervising PT.
• Assist patients in improving mobility, strength, and independence.
• Collaborate with patients, caregivers, and interdisciplinary teams.
• Monitor patient progress and communicate updates to the supervising PT.
• Complete documentation using AI-assisted tools.

Your Role as a Home Health Physical Therapist Assistant
