Concierge Home Care is seeking a Home Health Physical Therapist Assistant (PTA) in Jackson County, FL and surrounding areas. Earn $45–$50 per visit with a flexible schedule and autonomy to manage your daily plan of care.

You’ll provide one-on-one PTA services in patients’ homes, collaborate with families and care teams, and use AI-assisted charting tools to streamline documentation. Coverage area emphasizes minimized drive time with clustered scheduling.

Formación

• Active, unencumbered Florida PTA license.
• Valid driver’s license and reliable transportation.
• Home health experience preferred but not required.

Responsabilidades

• Provide hands-on care per the plan of care established by the PT.
• Assist patients in improving mobility, strength and independence.
• Collaborate with patients, caregivers, and interdisciplinary teams.
• Monitor progress and communicate updates to the supervising PT.
• Complete documentation using streamlined, AI-assisted tools.

Your Role as a Home Health Physical Therapist Assistant

• Provide hands‑on therapeutic care following the plan of care established by the Physical Therapist
• Assist patients in improving mobility, strength, and independence

Qualifications
