Job Description

The Physical Therapist provides competent, courteous, and respectful evaluation and treatment of patients for difficulties with mobility, function, strength, and positioning, adhering to APT scope of practice and licensure laws. The PT communicates and collaborates with families, physicians, and other health team members, maintaining professional documentation. The PT also conducts in-services and training for facility staff on restorative and functional maintenance programs. The PT reports to the Therapy Program Manager and the Director of Operations. CRI’s PTs model and promote professional services to develop and maintain a competitive advantage. They maintain career development through continuing education and networking. The successful candidate will demonstrate CRI values of Commitment, Respect, and Integrity in patient care.

Requirements
Essential Functions of the Job
  1. Evaluate Patients for rehabilitation services within 24 to 48 hours of physician’s orders.
  2. Develop effective treatment plans.
  3. Treat patients according to the treatment plan of care.
  4. Supervise the Licensed Physical Therapy Assistant and interact with staff, patients, and families.
  5. Communicate with physicians, managers, and team members regarding patient progress and plans.
  6. Participate in Plan of Care and Interdisciplinary Team Meetings.
  7. Achieve or exceed productivity standards.
  8. Participate in staff training programs.
  9. Record treatment and progress notes, and complete billing.
  10. Instruct families and staff in maintenance programs.
  11. Participate in discharge planning.
  12. Recommend equipment to facilitate clinical services.
  13. Report equipment issues.
  14. Act as a clinical supervisor for educational programs.
  15. Uphold Concept Rehab’s mission, vision, and values, exercising good judgment.
  16. Perform special projects and other duties as assigned.

Required Qualifications
  • Associates Degree in Physical Therapist Assistant.
  • Licensed as a Certified Physical Therapist Assistant in the state(s) of practice.
