Job description

Overview

Provides physical therapy interventions to designated patients under the supervision of a Physical Therapist (PT). Assists in monitoring patient progress through observations and follows up with the supervising therapist. Works under PT supervision as required under NY State Education law.

Responsibilities include:

1. Following established physical therapy plan of care, including interventions and patient/caregiver education. Modifying treatment procedures in accordance with PT plan of care and PTA scope of practice.
2. Documenting all care provided and maintaining records per VNS Health policy and standards.
3. Assisting in monitoring patient progress through observation and following up with Supervising Therapist as per VNSNY policy.
4. Complying with scope of practice for PTA, including co-visits with PT on initial visits and keeping PT informed. PT will re-assess every 6 visits or 30 days.
5. Managing assigned caseload and providing services as directed per PT plan of care.
6. Participating in VNS Health meetings, including interdisciplinary team meetings, in-service and training programs, rehab services meetings, and caseload reviews.
7. Traveling to patients’ homes and other facilities, adapting to varying environments, to deliver direct care.
8. Assisting patients with therapeutic interventions and activities of daily living, including positioning, transferring, lifting, and using durable medical equipment.
9. Providing care in accordance with agency policies and standards, which may require physical activities such as standing, stooping, and reaching.
10. Transporting and utilizing technology devices and supplies to and from patient locations and VNS Health offices.
11. Participating in special projects and performing other duties as assigned.

Qualifications

Licenses and Certifications: NY State Certification as Physical Therapist Assistant required. Valid driver’s license may be required based on operational needs.

Education: Associate's Degree as PTA from an approved NY State program or equivalent.

Work Experience: Minimum of two years clinical patient care experience required. Bilingual skills and basic PC skills may be preferred depending on operational needs.
