Job summary

One Home Health in Skokie IL is seeking a compassionate Physical Therapist Assistant (PTA) to join our home health team serving Skokie, Morton Grove, Des Plaines, Lincolnwood and surrounding communities.

The PTA provides therapy under a licensed PT, helps patients improve mobility, balance, and independence at home, educates patients and caregivers on exercises and safety, and documents progress for the care team. Flexible scheduling includes Full-Time, Part-Time, or PRN.

Qualifications

• Active Indiana PTA license.
• Current CPR/BLS certification.
• Valid driver's license and reliable transportation.
• Ability to travel throughout Porter County.
• Strong communication and documentation skills.

Responsibilities

• Provide PTA treatments under supervision of a licensed Physical Therapist.

Home Health Physical Therapist Assistant (PTA) | Skokie IL

One Home Health is seeking a compassionate and motivated Physical Therapist Assistant (PTA) to join our growing home health team serving Skokie, and surrounding communities.

Pay: $45-$55 per hour
Schedule: Full-Time, Part-Time, or PRN

Locations: Some location may include, Skokie, Morton Grove, Des Plaines, Lincolnwood, and surrounding areas.

What You'll Do

• Provide physical therapy treatments under the supervision of a licensed Physical Therapist.

• Help patients improve mobility, strength, balance, and independence in their homes.

• Educate patients and caregivers on exercises, safety, and fall prevention.

• Complete documentation and communicate patient progress with the care team.

Qualifications

• Active Indiana PTA license.

• Valid driver's license, reliable transportation, and auto insurance.

• Strong communication and documentation skills.

• Ability to travel throughout Porter County.

Preferred

• Home health experience.

• Knowledge of Medicare documentation requirements.
