Qualifications

Opportunities at 21HHS require being in the field visiting patients in their homes. We offer full-time roles such as Physical Therapist Case Manager, Level 2, with anticipated territory in the Hayward, CA area.

Responsibilities

• Perform an evaluation in the home, develop a care plan, and work with PTAs to follow the care plan.
• Help patients with mobility and independence, and create detailed treatment and care plans for each patient.
• Communicate and coordinate with patients’ physicians to ensure a quality level of care.
• Evaluate and report patients’ progress to the MD.
• Supervise 2 Physical Therapy Assistants (PTAs) to ensure they meet performance standards.

Qualifications

• Current unrestricted CA Physical Therapist License
• 1+ year experience supervising PTAs in a home health environment is a plus
• Current BLS/CPR Certification
• Valid CA Driver's License
• Knowledge of local community resources
• Knowledge of development, behavior, and how culture influences health care
• Knowledge of state and federal homecare regulatory guidelines (preferred)
• Experience in home health care and with high-acuity patients (preferred)
• Experience working with an interdisciplinary team; case management fundamentals (highly desired)
• Self-directed, empathetic, critical thinker with outstanding interpersonal and leadership skills
